Health Records and Immunizations

Health Records and Immunization Information

Healthy bodies and minds are the foundation for a successful school year. Make sure your child is ready by staying on track with immunizations and health forms. All children and teens should see their primary care provider at least once a year and their primary detal provider every six months. 
 
Immunizations

Under the School Year 2026-2027 Immunization Policy, students in the following grades must be up to date on all required immunizations by December 7, 2026:

  • Pre-K 3 
  • Kindergarten
  • 7th grade 
  • 11th grade 

Students not in compliance by December 7, 2026 may not be allowed to attend school, unless they have a medical or religious exemption on file. Learn more about which immunizations are required here, and locate a clinic here.

Schedule a Health and Dental Visit 

Although forms are accepted at any point throughout the school year, families are strongly encouraged to submit the Universal Health Certificate and Oral Health Assessment Form at the start of the school year. Make an appointment with your child's doctor and dentist today to ensure they are ready to start the year strong.
